Slide notes:Individually identifiable health information can be divided into three main categories. The first  category, physical or mental health condition, refers to  information that relates to a persons past, present, or future physical or mental health condition. The second category, provision of health care, describes the health care services that have been or will be provided to the person. The third kind of protected information is payment information. All of this information is protected as long as there is a reasonable basis to believe that it can be used to identify the individual. 12
